Reassess cross domain mappings as new data arrives #46

Basically run the vocab generation code and examine the domain_ids that come out of certain CCDA sections, cross check with mappings.

this really means adding columns to the task matrix.

The thing to note is that the section_code_snooper pulls ALL codes from the section and it's not just the _visit_concept_id. For example medications in CCDA include observations after the medication is administered, and those codes would be included in this extraction.
