26 ner readme v3 #69

fixes #26 Because this moves the guideline that already existed into the readme. both parts are satisfied.

This branch takes changes from this PR https://github.com/clamsproject/aapb-annotations/pull/63 and this branch, and combines them with additional changes.

For reference, for NER this will complete the related #26 issue. as the process.py is finished.

This branch also made changes to the NEL files, for that #25 issue, the process.py summary is incomplete.

I have reviewed the sections that were changed by Keigh.
