Multiple improvements to the 2d/sloping_fault example. Includes:
1) changes suggest in clawpack/seismic#6
2) notation change in dip angle to match standard convention (between 0 and pi)
3) made regions specified in setrun dynamic to changes in fault parameters
4) removed fdisc.f and files in Makefile that were carryover from when the initial condition wasn't zero
