Improve C implementation

[mt-clearhaus]

• Combine ciphertext and tag parameters
• Compile with -Wall
• Hardcode iv rather than dynamically allocating it
• Allocate less memory for plaintext
• Do a segmentation fault if we have written beyond plaintext
• Reverse the order of the cleanupX labels
