Switch to default WP cookie naming convention for session and state cookies #240

Fixes Pantheon.io and all other Varnish cache bypasses that employ the default wordpress_ prefix cookie naming convention in their VCLs

This is great! The only issue that I see here is that we need to migrate our old session to this new naming, otherwise users who were logged in with Clef will not be logged out (that state will be lost when the plugin gets upgraded). I can write this migration!
