RegionType annotation NOT qualifier rename to outcomeQualifier #205

Note that this will be the exemplar for using the "qualifier" pattern. All others are being reduced to provide explicit "not xxx" values in their value sets.

This is mostly due to the fact that we have some examples and because we all agree that it is an important use case.

@bpow I am making this modification to the spreadsheets now.

renamed and re-associated the general "statement.qualifier" attribute to be "RegionType.outcomeQualifier". This is currently the only use of the "qualifier" pattern in our examples based on our discussions and decisions from the 4/10 meeting with @bpow @mbrush and @cbizon .
