Moving outcome and outcomeQualifier to Statement #218

moving these attributes to the Statement parent class in conjunction with remapping the ValueSet bindings (see #217) so that all sub-classes of Statement have a consistent attribute for the "outcome" as well as the "outcomeQualifier".
