Complete the transition from datamodel.clinicalgenome.org -> dataexchange.clinicalgenome.org #240

May need to check dependent projects (e.g., VCI interface) to make changes there.

Should have something at datamodel.clinicalgenome.org that redirects or at least has some text referring to the new location (since the old URL was part of abstracts for prior presentations).
