Add Tracker tab for Glaucoma VCEP Report #58

@RyanFWebb I originally opened this for the Glaucoma VCEP, but added a few more

The Glaucoma tracker is good to go.

The Monogenic Diabetes tracker is good to go. I tried to copy over records to the tracking tab for them, but I wasn't sure what to copy in all cases, so I left them an "OLD priorities w/ notes" and "OLD tracker" tabs for them to refer to until they are ready to delete them.
