Structure compound variants

convert variation lists within an assertion to a tree structure for compound variants
add cloudbuild
refactor massive sink.clj to pull out clinvar_combiner specific functions to that module, and some others into util.clj

NOTE: includes no-op main merge due to strange PR merge resulting in mismatched commit hashes
