Initiate real-time ClinVar VCV data stream #281

Use the ClinVar combiner data stream to provide a continuous, regularly updated flow of VCV data on a separate data stream with the minimal amount of data to support the Baylor VPT (Variant prioritization tool). The stream will explicitly represent new, updated and deleted VCVs such that reading the stream from the beginning to the last offset will reproduce the current set of VCV records as contained in the latest DSP consumed ClinVar variant data release.

@theferrit32 I think we should move this to the backlog for the time being. Since we are shifting our focus to getting the SCVs created, I feel like we can revive this epic after that. What say you?
