fixed issue #147 and change our DBHelper to thread safe singleton. #178

  1. rename the column "Quantity Dispense to Facility" to "Quantity Dispense" .
  2. Calculate the "Quantity Adjustment" as add up all adjustment.
  3. change LmisSqliteOpenHelper to thread safe singleton.
