lmisqa.dhis2nigeria.org.ng should be installed at root #232

Currently, http://lmisqa.dhis2nigeria.org.ng/ is installed in the /dhis subdirectory - meaning the rest endpoint for the apis is at /dhis/api instead of /api

Normally, this wouldn't be a problem, but the other environment (e.g. lmisproduction) is not in the /dhis subdirectory. This means if we do a DNS swap, and point lmisproduction to the qa server and vice versa, any clients using the production URL will fail once the DNS is pointed to the new server.

For consistency, all servers should be installed at the same rest endpoints, especially if there will be more swapping in the future (I hope not, but who knows...)
