Join page: consider changing format of page and adding contact form #166

Right now the page has a lot of text. Could consider breaking up the page and adding a contact form. Wayne, maybe we can check in on this next week.

Contact forms can be created in the "Contact" In the navigation and embedded with a shortcode. If you have a sketch of what you would like to do, I'll do my best to get it finished before launch.
