Fix the Contributor Agreement e-form link format #385

Context of the problem

The Instructions for submitting the agreement section of the Contributing to ClojureScript page has a format problem:

It formats the link and removes the bullet list. If the bullet list is needed/wanted, feel free to edit these changes!

I just read the Contributing to This Site page and noticed that it says that if I wish to contribute, I must become a contributor and sign the Clojure Contributor Agreement before submitting changes.

So I opened the #386 issue for describing the problem, and I am closing this PR.
