Update User Model Management features and Swagger API docs

• Update User Model Management features
  • Update API URL : /cloudinfra/:id" -> /onpremmodel/:id
  • In order to include not only Infra model, but also S/W model and Data model in the future
  • Add more parameters such as
  • OnPremModelVer in case of On-premise Model
  • CloudModelVer and IsTargetModel in case of Cloud Model
  • Rename parameter names below
  • Name -> UserModelName
  • Version -> UserModelVer
• Update Swagger API docs
• Update README.md
  • Add Notice of Log file path
