Add TOC functionality

In order to improve the navigation of pages with lots of headings (e.g. https://cloud.gov/docs/services/relational-database/), this PR experiments with adding a table of contents to the top of the page using the jekyll-toc plugin.

TOC is enabled on all _docs entries. Examples:

• Lots of headers: https://cg-88d42ca6-59d7-47e0-9500-4dd9251360b9.sites.pages.cloud.gov/preview/cloud-gov/cg-site/add-toc-functionality/docs/services/relational-database/
• No headers, No TOC: https://cg-88d42ca6-59d7-47e0-9500-4dd9251360b9.sites.pages.cloud.gov/preview/cloud-gov/cg-site/add-toc-functionality/docs/overview/customer-service-objectives/

None, just hopefully improving the UI/UX of our docs
