Cp4d healthcare pattern bootstrap #270

Create a new bootstrap just only for CP4D demo, go back bootstrap to original and create a nuw set-git just only for CP4D healthcare demo. To Hollis: I don't have any branch for testing here so I'm asking the merge for master. Please let me know if I have to do something more
