Various moves, renames, and fixes

This PR includes resolutions for #262, #246, #267, and #268.

Also,

move some classes to more logical locations
improve documentation and error logging
decrease responsibilities of the classes in c5-general-replication
enable fsync (FileChannel#force) in the replicator log
rename interface ModuleServer to ModuleInformationProvider
repair some tests that were failing because they didn't have the proper jmock expectations (BasicTableCreationTest, OLogShimTest)
check netty channel futures for failure in ReplicatorService
change QuorumDelegatingLogConcurrencyTest to run in memory rather than using disk (greatly improves speed)
better typing for some constants (e.g. use Path instead of String)
