<button><img> alt-text not read #94

If there is an image inside of a button (yes, the weird input element), then 
it's alt-text should be read along with the other content (if any) of the 
button.

If there is no alt text, then the image should be spoken as "image."

If alt="" then nothing should be read for the image inside the button. Other 
content would be read.
