Add support for multiple ports in lattice-app #7

Changes to lattice-app to support listening on multiple ports.

Multiple ports can be specified as command line arguments --ports=port1,port2,...
If no command line argument is specified then it reverts to its original behavior, which is to use PORT env if set else default to 8080
This PR doesn't involve any changes to docker image. It should be built and pushed to docker hub after this PR has been merged.

Thanks for incorporating my feedback to using command-line arguments for now.

Merging. I'll need to fix the repo to import using the proper new org name cloudfoundry-samples. I'll also docker build / docker push the new image .. soon. Keep you posted.
