Clarify that log rate limits can be specified on tasks as well as apps
Remove unused reference to log-cache plugin from firehose plug-in example

We have created an issue in Pivotal Tracker to manage this. Unfortunately, the Pivotal Tracker project is private so you may be unable to view the contents of the story.

Thanks, @acrmp. Which TAS versions should be updated with this change? I've done TAS 6.0. Please let me know if it goes further back.
