After the first two title pages and disclaimer on page three, the front matter of the report should be structured as follows:
Page 4: Request for Contributions
Page 5: Reports on Computer Systems Technology; Abstract; Keywords
Page 6: Acknowledgements
The attached document contains the standard text placed in all volumes for the Request for Contributions and Acknowledgements. The Acknowledgements text should be used at the beginning of the Acknowledgements and should be followed by a list of the contributors. The other volumes have contributors in three columns on the page, since there were many names. The formatting of the names is bold, 10pt font, Times New Roman. The person's employer is listed directly underneath the name in 10pt font, italic (not bold), Times New Roman, with no extra line space between the name and the employer. Between the employer of one person and the name of the next person there is a 5pt line space.
In addition, the attached file contains text for Section 1.1, which is the same in all volumes. Please copy it verbatim to Section 1.1.
