From this report, this would be an indicator that the extension has not loaded correctly. We'll look into this here in an attempt to correlate telemetry data with what you're experiencing.
It would be helpful if we could determine:
Update: We were unable to find any relevant telemetry, again, a likely side effect of the extension not loading correctly (thusly it doesn't know how to send telemetry data back).
Have you tried installing the extension and then reloading VS Code using the developer reload command? Or close out VS Code entirely after installation?

To find the CDS for Code output window, start by opening the integrated terminal
Then click the output tab, making sure to select "CloudSmith CDS for Code" in the dropdown
It would be helpful for us to see what was logged in there. If we can't come up with something from that, I would be happy to schedule a remote debugging session with you if you'll have the time.

I can't see CloudSmith CDS for Code listed in the Terminal.

At this time we are unable to reproduce this error on our local builds. If you have availability, we can set up a screen sharing session to step through this on your machine to triage the problem. To do so, please send an email to support@cdsforcode.com referencing Github issue #14 and we will be happy to coordinate.
We would love to spend some time to get this resolved.
When i am trying click on + sign to add a connect,i am getting er: command 'cs.cds.controls.cdsExplorer.addEntry' not found. Reinstalled and still getting same issue.
