Documentation Update #11

When following the README.rst and USAGE.rst docs, I stumbled upon several inconsistencies and minor errors which took a while to track down. This PR:

Fixes the minor errors.
Makes the formatting more consistent, particularly for the RST format (much was written for MD format).

This got merged in a funny way due to a conflict and my resolution and somehow performing enough git magic to also sign the commits with my gpg key while maintaining attribution to the original author. I'm closing this manually since git can't figure that out on its own.
