Remove deprecated code (January 2016)

I have marked a number of items as deprecated and promised to remove them on or after January 31, 2016. The actual milestone will be once we have done two or three production releases at $work without any drama. I'll keep a list here of items that are going to be removed:

• Test::Clustericious - is buggy and not widely used
• Config files with .yml extension - not widely used
• Loading config from a scalar reference - only used in tests, and there are better ways to do this now
• dump_as_yaml method in Clustericious::Config.
• JSON configuration files - only used in tests as far as I can tell, and JSON can fully be represented in YAML anyway.
• Remove clustericious generate mbd_app command (it already returns an error now)
• arguments as hash in Clustericious::Command::start
• using simple_auth in config file will croak for now. Along with this set of deprecations being removed, the code to check for simple_auth should be removed.
• Clustericious::Controller#render_text and #render_json (these were compatability subs in Clustericious for older mojos which deprecated and removed them a long time ago)
• base_tag helper from Clustericious::App
• auth_ua helper from Clustericious::App
• calling init_logging as class method (Clustericious::App)
• Clustericious::RouteBuilder::Proxy , also there is some proxy specific code in Clustericious::App and possibly other places.

I may add more before the winter break.

We can probably loose the dep on List::MoreUtils after these are removed. mesh is used in Clustericious::Command::start in code that is scheduled to be removed. uniq is used in Clustericious::App, but the size of the dataset probably does not warrant the dep on List::MoreUtils

• arguments as hash in Clustericious::Command::start

should go in the February list of deprecations since many configs still haven't been updated yet.

May also need to delay this removal:

• calling init_logging as class method (Clustericious::App)

as it is used by a number of commands still.
