1. Make a call

seperatly.

Which SIP server are you using? What happens with PBXes?

I have the identical problem on my Motorola Atrix (android 2.2).
1) All calls (in and out) seem to default to the back speaker & microphone as
opposed to the usual front earpiece speaker and mic.
2) Turning the "Speakerphone" on during a call just seems to increase the rear
speaker volume level.
3) (maybe unrelated) I am also experiencing some wierd audio "buffeting" at
some point during a call but I don't think it's related to this issue. Seems to
be identical on wifi, 3g, or edge.
