Rename PR template so it is properly detected #25

The pull-request-template should automatically populate the description of a new PR. That's not working as expected. I've encountered this before, GitHub's documentation is not very clear.

This renames the template in order for it to populate new PRs as expected.

I'm going to merge this without any approvals to have it in place for future PRs.
