Phase 2 GTT fix L1TrackObjectNtupleMaker extended track indices #1255

PR description:

This PR extends the index fix applied for 4-parameter prompt tracks (https://github.com/cms-l1t-offline/cmssw/pull/1234) to the extended (5-parameter) track collection

PR validation:

This PR passes: scram b scram b code-checks scram b code-format cmsRun L1TrackObjectNtupleMaker_cfg.py
