Introduce a suggested action system #18

I won't be able to support code actions because of the way they break the experience in a Live Share session -- some folks pair and mob this way all day. So, I had to come up with an alternative that would provide the same kind of functionality.

Currently, the suggestion system is in place, but it only suggests refactorings. There are still the following to complete:

conversions
general code actions

This issue is here merely to track progress
