KCDs + Accessibility Questionnaire #526

Catherine Pagini and the https://contribute.cncf.io/about/deaf-and-hard-of-hearing/ suggested that we start having KCDs ask when people register if they need accessibility support. This can give us data to see what CNCF can support KCDs in this area.

I think adding a question: Do you need any accommodations? yes/no. If yes, what type of accommodations do you need? Would be great.
