Create Governance Review Template #321

Given https://github.com/cncf/toc/issues/963, it's time to develop a governance review template for graduating projects.

We can base this on what we've done for Cilium and KEDA.

The template has been implemented, but we need to look at the governance reviews for the people using the template and see if we need to make any adjustments.
