Documentation generator updates #1191

Motivation:

the need to support documentation generation for both .coal and .lisp files in #1180
the desire to provide a documentation interface to emacs via SWANK
the lack of sort ordering on the website

Changes:

remove direct dependency on ASDF
add BIG-FLOAT to documentation output (Closes #722)
sort packages, types, classes, instances, etc. (Part of #886)
use git rev-parse to link to exact source version
general refactor to merge duplicated code, separate data access from model and output routines
