This PR creates a new global tool menu on the right hand side of the Quarto HTML page similar to what is offered for Including Other Formats and Code Links and Other Links.

This is great and opens the way for more functionality in the future. I think it would make sense to be able to control whether context: setup blocks are included in the history and maybe exclude them by default.
