Add more details to the shortcode parameters section to make it more clear #67

This also fixes:

  1. The double-quote mismatch in the shortcode example
  2. The problem if you add the logged_out="0" parameter

I also added a note to help clear up the confusion with restricting media attachment pages versus media server files.
