since when? what version & browser (/youtube changes? did you try logging out or incognito window) (/did you notice the bug report template?)
uhh so i just got this from the opera extension store i think 2-3 days ago, i am using opera gx currently, i cant remember if i tried relogging for my issues that i got improvetube tofix or after some my saved settings werent being applied. and im not sure i kknow what template you mean so im assuming no. these settings worked as intented that first day, but i believe since either an opera and/or computer restart its been a little wonky. im not sure of many more issues bc my biggest priority and one of the only things im noticing as "wrong" is i have suptitles enabled in the extension but i have to manually turn them on each video. same thing with autoplay but i think i like it off now
actually i turned autoplay off and then videos couldnt be unpaused. like hitting play just made it unpause for maybe a frame, i then switched it back on and it allows the video to pause and resume normally
that sounds familiar (guess you reloaded the page?) and some things we have to do might be more tricky than it sounds... (next to youtube changing),
yet we cant guess if this bug happens to "only 100" people per day or 100 000, other than looking for more reports, more details, browser console errors, DOM elements, extension errors chrome://extensions/?errors=bnomihfieiccainjcjblhegjgglakjdd
(- And/or we could somehow raise funds, just enough to hire developers in poor/cost-efficient places.)
update: its has miraculously fixed itself

Using latest public version of Firefox and i'm having this same issue. The only way i've found a consistent and persistent fix (for a time) is to go into Firefox Settings> Privacy and Security> Cookies and Site Data> and click the 'Clear Data', select 'Cookies and Site Data' and 'Cached Web Content' and then click clear. Though, i believe it's the 'Cookies and Site Data' is where the issue lies. The reason for this is i've tried clearing just 'Cached Web Content' alone made no difference. Other things i've tried is clearing data manually in Firefox settings, clearing Cookies for Youtube using other extensions, clearing them with this extensions option, reset this extension settings, and re-configuring just one setting (playback speed), disable/re-enable the extension, and finally restarting Firefox. So whatever is causing settings to be "forgotten" is in 'Cookies and Site Data'. Hope that helps in some way as i've run out of ideas...
EDIT: Wanted to clarify that i mostly use the extension to force a persistent playback speed but have toggled a few other settings such as force theater mode (which i think persists by default) and noticed that my custom color appearance settings were still working. I've since tried a few other settings such at Quality which also seems to be working. So it seems for me it's just Playback speed not sticking. Another odd thing i've noticed is that when the extension is fully working 'Autoplay' is enabled and it seems to function normally, but when the extension is not working, disabling 'Autoplay' causes there to be a sort of auto-pause function as soon as play to be hit. So if i hold down play (k) the video plays but with a continuous hiccup and flashing play icon.

if you think it only happens in this Firefox version, please report it to Firefox too. (Same for Opera GX, especially as it is younger than other big browser @AlbinoMann)

It should always work! Please check F12
-key, browser console errors before and when all of this happens. Please check repeatedly to see if there is variation in the error or not.

that was a bug couple months ago while trying to fix Autopausing, its fixed now afaik

Certain setting, like the progress bar color are working fine, but autoplay and subtitles are checked on and worked on first install but upon restart these features are behaving as if they arent checked and i have to play videos and activate subtitles manually. deleting my settings and reseting everything did not resolve the issue either
