Fix undefined array key issues and make the Form class working under Contao 4.13 #167

There are 2 commits: First commit will remove a few "undefined array key" issues.

The socond commit will patch the "missing/invalid request token" issue. Since contao 4.13 the request token ist no more longer set via the {{request_token}} tag in the form_wrapper.html5 template. For this reason we have to set the request token in the form class.
