Since neighborhoods are often organized by 'bigger area' then medium and smaller areas, reorganize the neighborhood taxonomy to allow for the first level hierarchy to be selected, and then additional sub-neighborhoods.
But also allow for someone to select non contiguous neighborhoods (so perhaps going backwards and forwards with javascript, and having the selected neighborhoods appear on the side (outside of the select box.)
