Update question survey scales: add more in-between options #107

@ezbycer @jenningshanna

To ensure that our data most accurately represents user behavior, we need to add more options to the survey scales. This ensures that the data is not skewed simply because users didn't have enough selection options presented to them.

SELECTIONS TO BE ADDED

Never, rarely, sometimes, often, not sure

Very little, some, most, all, not sure

Everyday, Every Few Days, Weekly, Monthly, Not Often

A Week,One Month, Two - Three Months, Six Months or more, Not sure

@boonrs I am not sure how this will affect anything on the back-end.

@boonrs To what extent would this change make new instances relatable to the data from orgs who've already gone through the pilot? Could the same spreadsheet be used to capture this data as the one already in use for the current pilot?

My interest is trying to get a sense of what impact this would have, how long this would take, and ultimately if this feedback (which is important) should take precedence over some of the other items we've discussed.

This would currently have no impact on the code.

This would have impact once #58 is implemented. Additional items or a change to the answers would change calculations.
