Fix donate links #78

Hi Brie, can you please change the donate button link on the Brigade nav, so that it goes to: https://secure.codeforamerica.org/page/contribute/default?source_codes=brigade-nav-donate-button

And the Donate link in codeforamerica.org/brigade/about, so that it goes to: https://secure.codeforamerica.org/page/contribute/default?source_codes=brigade-about-page

I don't have admin rights for this repo, and I believe you do, which makes it much easier to make quick edits like these - thanks so much!
