Per the discussion sort of coming to concrete terms here https://github.com/codeforamerica/cityvoice/pull/132#issuecomment-35595432

I think this might be the lightest-weight way for redeployers to configure a survey.

The ~~.yml~~ CSV file would have a set of questions with the following attributes:

Voice file URL
Text representation of the question (for presentation in the view)
Order # (as in, it's the first question in the survey)
4 "response_text_N" fields (e.g., response_text_1: "Beer Revolution")

Totally done as a CSV importer in https://github.com/codeforamerica/cityvoice/pull/151. Not sure what the survey format is for the response_text fields, since they don't exist in the app right now.
