Brand as CfA #148

Doesn't necessarily need to be in the logo or anything, but I think we should include some CfA branding somewhere to distinguish between HSA/government services. Also see https://github.com/codeforamerica/calfresh-and-so-clean/issues/145

Not sure this needs to happen today. We are working with assistors in the Code for America building. We've updated language to avoid referring to "we" or "us"

@daguar @lippytak I think this is addressed by the open pull request #181. Lmk if it needs further attention.

I'd actually like us to step this back up — at least maybe having something at the bottom on the first page, and at the end. The "Clean Assist team" branding is okay, but I'd like to explicitly call this out if we can.

I think the obvious approach is to include a footer, like every other website in the world. This is related to #352 as well.
