@cydharrell would love your feedback on this if you have the time! The goals are in the original issue but basically we're trying to assess the challenges with the CalFresh application process and identify possible solutions from the CBO assister perspective. In particular I'd love to learn if there are any standard survey strategies to get at "what are the challenges" and "what are possible solution" issues.

• no 'number' field because allows negative :(
• condense some options (aware, apply, FU) or something
