Current Workflow
Virtually every navigator we tested with wanted to use the app to make referrals. For navigators, screening is just one step in a referral process. To do a referral, they described several steps:
- Deciding which services and locations should be referred to.
- Entering the patient info
- See what the patient is eligible for, and explain the next steps to the patient
- Respond to any feedback about the referral (make corrections, add missing fields)
- Follow up to see if the referral worked.
Specific issues in the current workflow and possible fixes:
- In the current workflow, navigators are given more context about services and eligibility results when doing prescreenings than they are with full screenings. --> add service details, service selection, and service eligibility results to full screening
- Assisters expressed a strong need for more clear info about services to allow them to better filter which ones they should send patients to and to better help a patient with the next steps. --> Allow navigators to access service details at multiple stages in the screening process
- Assisters expressed a strong need for feedback about the success or failure of their referrals, particularly when doing a full screening. If they're going to enter all the info, they want to know that someone received it. --> Possibly add an email notification for feedback & rejection about referrals.
Proposed new workflow for navigators making referrals:
In this proposal, note that prescreening and screening are merged into a similar workflow. The only difference between them would be the number of questions and perhaps the ability to send a completed application to a service.
This is for discussing the overall UI workflow for different user needs and tasks.
