Regarding #98, Medicaid and insurance questions have Not Sure answers, but because the values are blank "", they result in "not eligible" on the results page. Didn't touch back end. screen shot 2015-05-20 at 6 01 58 pm screen shot 2015-05-20 at 6 02 10 pm

regarding #125, I put limits on DOB so you can't be born in the mid 1800s, or more than a few decades in the future. Didn't edit the household member forms because these seem to have a mix of hidden inputs and inputs named _age so I wasn't sure what to do there.
