Fix access now validation errors #188

I hadn't understood the validation settings clearly enough on the Access Now fields I added. This should fix a lot of that.

For service-specific questions: if providers list is not manually added, the default will be that no additional classes are added. If any providers are manually designated, the field will get a q_unique class in addition to the service-specific classes (q_an, q_dp)
