Prescreen patient details #189

Runs the prescreening logic on the data in patient details.
Adds the results to the patient sharing tab, using the same template used for the standalone prescreening results.

@esmithayer I'll get this branch synced with next-visit-updates-183 once the merge conflicts are fixed and make a PR so we can all be on the same page. Thanks for putting this together!
