Adds super basic email notifications when a user sends a new referral to a service.
Adds the ability to associate one or more email addresses with a service for the purpose of receiving referral emails.
When a user clicks "Send Referral", it sends an email to any email addresses associated with that service.
The email contains the name and organization of the user who created the referral, and a link to the patient's profile. (Not sure about the HIPAA implications of including more info about the patient?)
The mock data script now associates all services with richmond@codeforamerica.org by default.
