Adding styles to @esmithayer's addition of document thumbnails on the patient overview page :camera: :page_facing_up: :scroll: :+1:

note: the "add new document" button just links to the #documents anchor on the patient details page.

Also adding styles for past screening results to the patient overview page, which show up above the form.
