Bug fixes

Fixes the following bugs/things/what-nots

• Updated race/language questions to be checkbox answers, which includes an update to the conditional formatting scripts
• logo is a link to the index page
• empty currency fields were marking invalid if empty, this no longer happens
• <ul> for edit history
• dirty form save button checks on keydown as well as change
• fixes a broken link in the footer
